Description:This is a treatment manual for professionals who are dealing with MISA (mentally ill, substance abusing) clients. Since providers are usually trained in one of these areas, this book will help acquaint them with both areas and how to make effective interventions.
Purpose:According to the authors, "by the early 1980's, it had become apparent that the mental health and substance abuse treatment systems had numerous clients. The purpose of this book is to examine how these dilemmas have been addressed, the treatment approaches that appear to be most effective for clients with multiple disorders, and how best to manage multifarious caseloads composed of individuals with and without co-occurring disorders." The book definitely meets the authors' objectives.
Audience:It appears clear that the book is written for practitioners and administrators working in the fields of mental health and/or substance abuse, but graduate students in clinical psychology could certainly benefit as well. The authors are credible authorities. Edward Hendrickson has been in the field since 1971 and Marilyn Schmal since 1974. Sharon Eckleberry serves on the Licensure Board for Social Work in Virginia and has authored articles on dual diagnosis.
Features:The book covers extensively dual-diagnosis issues. The authors begin with their philosophical viewpoint, the TAUT-SOAR Model, which gives practitioners a framework to build on to. It covers assessment, treatment, and case management. There are good chapters on measuring change and supervising staff with multifarious caseloads. The approach is pragmatic and healthcare providers can readily apply the concepts. The book is easy to read and covers the basics while the many case examples make it easy to learn.
Assessment:The number of MISA clients is growing exponentially. This book covers many of the most important areas in this field. The authors do not give simplistic answers and they understand the difficulty of working with such clients. In the chapter on promoting long-term stability, they provide this optimistic viewpoint: "The majority of this work is done in the early recovery and treatment-engaged phase; however, in some instances, important basic knowledge and skills must first be introduced before recovery can begin, and life crisis events may require additional recovery-management interventions after recovery is well established. Any return to substance use or psychiatric instability should be viewed as an opportunity to strengthen recovery skills rather than as a treatment failure" (pp. 177-178). It is in this spirit that I give the book a hearty thumbs-up.
