Reich, MD(University of Maryland School of Medicine) Description: The comprehensive scope of the third edition of this book is reflected in an opening chapter on the history of medical therapy for Parkinson's disease (PD) and a closing chapter on neural stem cell therapy. In between are scholarly, well-balanced chapters on all currently available medical and surgical therapies for PD. There are equally high-quality chapters on imaging, non-motor symptoms, nutrition, physical, occupation, and speech therapy, as well as nice coverage of emerging/experimental therapies. Particularly appealing and user-friendly is the division of chapters into an outline format; most have a well-written, balanced, "take-home message" conclusion. Purpose: The editors have, with few exceptions, succeeded in their goal of providing a thorough, up-to-date reference for physicians and other healthcare professionals who care for patients with Parkinson's disease. A new edition is well-justified in light of recent advances in medical and surgical therapy of PD. Furthermore, this book is sensitive to the growing complexities of caring for patients, such as deciding on initial therapy, determining who is a surgical candidate, the impact of non-motor symptoms on quality-of-life, and complications of advancing PD, including dementia and psychosis. Audience: The book is targeted primarily to neurologists, but its broad coverage makes it an equally valuable resource to almost everyone who cares for patients with PD, including neurosurgeons, therapists, and primary care practitioners. The editors and authors are recognized experts and this is reflected in the authoritative, comprehensive, and well-written chapters. Features: A chapter is devoted to each of the currently available medical therapies for PD; separate chapters cover ablative surgery and deep brain stimulation at various sites. A novel feature are the chapters on subjects that are usually inadequately covered, such as electroconvulsive therapy for PD, radiosurgery, and transcranial magnetic stimulation as well as nutrition, physical, occupational, and speech therapy. Non-motor symptoms are addressed in chapters on autonomic disturbances, sleep, psychosis and dementia, anxiety and depression. Practitioners needing advice on treating drenching sweats, sensory symptoms (especially pain), and akathisia in PD will, surprisingly, not find it here. Other shortcomings include the lack of a chapter devoted to management of motor fluctuations, inattention to the explosion of information about quality of life in PD, and caring for caregivers. Assessment: If you have a previous edition of this book, you'll want to replace it with this edition. If you don't have a previous edition and care for patients with PD, this will be a welcome addition to your library. James Parkinson wrote in his historic Essay on the Shaking Palsy, published in 1817 (1), that there was "sufficient reason for hoping that some remedial process may ere long be discovered, by which, at least, the progress of the disease may be stopped ... although the removing of the effects already produced, might be hardly to be expected."
