The Book Seems Weighted Towards Clinicians Who Deal With Newborns, Particularly Those Who Deal With Respiratory Problems. It Will Be Helpful For Those Who Care For This Population, Particularly Neonatologists And Pediatric Pulmonologists In Clinical Practice. It Lacks The Depth That Researchers And Super-subspecialists Would Probably Desire. features: the First Part Of The Book Reviews The Normal Physiology Of Respiratory Control In The Fetus And Newborn. By Necessity, There Is Extensive Discussion Of The Developmental Aspects Of Respiratory Control. Both Systemic And Cellular Aspects Of The Topic Are Addressed. Since This Topic Is Closely Allied With Sleep, That Topic Is Also Covered. The Development And Physiology Of Neuromuscular Innervation And Control Of Respiration As Well As The Chemical And Metabolic Features Are Included In The First Half Of The Book. The Second Half Is Devoted To Disorders And Diseases Of Respiratory Control. The Topic Of Apnea Of Prematurity Accounts For About Half Of This Part, With Several Chapters Covering Various Aspects Of The Pathophysiology, Diagnosis And Treatment Of This Disorder. The Remainder Of The Second Part Consists Mostly Of Single Chapters Addressing Other Disorders -- Bronchopulmonary Dysplasia, Congenital Central Hypoventilation, Gastroesophageal Reflux And Sids Each Have A Chapter. The Broader Topics Of Neuromuscular And Airway Anatomic Disorders Are Addressed In Rather Brief Chapters. The Book Has Both Subject And Author Indexes, A Feature Which Can Be Very Helpful If One Wishes To Follow A Particular Author's Train Of Thought Across Several Topics. Each Chapter Has Separate References Which Are More Than Adequate For The Clinician, But Probably Not So For The Researcher. With Multiple Authors Addressing Similar Topics, There Are Bound To Be Contradictions And Dissent. On The Issue Of Apnea Of Prematurity, There Are Authors Who Seem To Disagree About Whether This Is Primarily A Disorder Of Central Respiratory Control And Apnea Versus Hypoxemia Due To Hypoventilation. Both Sides Make Good Points. Which Side Is More Persuasive Will Likely Depend Upon Whether The Reader Is A Researcher Or Clinician. Curiously, The Book Does Not Include A Separate Chapter On Diagnosis And The Techniques Of Polysomnography, Multichannel Pneumocardiograms, Esophageal Ph Studies, Etc. Each Of These Techniques Is Discussed By Individual Authors In Various Chapters And There Is A Chapter Devoted To Monitoring In The Nicu. However, The Reader Would Benefit Greatly From A Unified Discussion Of The Role Of Each Diagnostic Modality. The Sensitivity, Specificity, Predictive Values, Reproducibility, Risks And Costs Of The Various Techniques To Diagnose And Monitor These Disorders Are Important Issues For Both Researchers And Clinicians.
