Much has happened in the field of Rheumatology since the book's previous edition was published five years ago; this most recent edition, the book's fifth, contains new chapters detailing the many areas of advancement in the field of Rheumatology. Purpose: The general purpose of the book is to deliver up-to-date, comprehensive information about the field of Rheumatology in a user-friendly, highly readable format. The new edition is particularly timely, as it integrates many of the recent advances in our understanding of the basic science of autoimmunity, as well as our expanding pharmaceutical options for patient treatment. The book meets its authors' objectives: it is well organized, easy to read, relatively comprehensive, yet can fit easily in the pocket of a white coat. Audience: The book is primarily written for students, residents, and fellows, and it does contain the practical information one would want in a manual, for easy access on the wards or in a clinic. Moreover, many experienced practitioners, of Rheumatology, Orthopedics, and other fields, would benefit from having this manual close at hand. The comprehensive formulary (more than 50 pages), lists of ACR criteria for particular diagnoses, and detailed diagrams of physical therapy exercises for specific musculoskeletal complaints, are particularly useful. The editors of the fifth edition of this book are all highly respected experts in the field; most of them are from the Hospital for Special Surgery in New York. In addition, the authors of the individual chapters are acknowledged experts in Rheumatology, and particularly in the area of their contribution to the book. Features: The book is well organized and comprehensive. The first third of the book focuses on the general rheumatologic approach to the patient, and to specific musculoskeletal complaints. The remainder of the book details the specific diagnoses that comprise the field of Rheumatology. The authors strive to present concise discussions of our current knowledge of etiologies of the various diseases, as well as practical recommendations for diagnostic and therapeutic approaches. All chapters are carefully evidence-based (or acknowledge the lack of current evidence to support particular clinical practices). Particularly useful is the "Key Points" section which precedes the text of most chapters, serving to point out the most important points (in bullet format) for the reader. Assessment: The Manual of Rheumatology and Outpatient Orthopedic Disorders is a handbook of the highest quality. It is not, nor does it strive to be, a comprehensive textbook. Rather, it is a useful, readable, portable reference suitable for trainees and beyond. It is more detailed, and more authoritative, than other Rheumatology handbooks I have read. In particular, a quick glance at the authors of each chapter shows them to be acknowledged experts in the field; they are not merely summarizing previously published manual chapters, but are often presenting the most current pathophysiology, diagnosis, and treatment data available (data to which the authors often contributed substantially through their own research efforts). Though I have not read the fourth edition of this manual, the fact that five years has elapsed since its publication provides justification for this new edition, given the rapidity of the advances in rheumatology in recent years.
