Despite lifestyle improvements, the incidence of rectal cancer is increasing in industrialised countries. Rapid advances in technology, growing knowledge of the biological history of the disease and closer attention to patients' quality of life after surgery have led to a less invasive approach. In the last 15 years, the surgical approach has shifted from extended resection to sphincter-saving procedures, featuring a multidiscipinary approach and a high level of specialisation. The experienced surgeon can plan and choose the "right treatment for the right patient" only with the support of the radiologist, endoscopist and pathologist (preoperative staging), oncologist and radiotherapist (neoadjuvant therapy), and psychologist and stomatherapist (rehabilitation). In addition, the difficult problems of salvage procedure and the reconstruction of anal sphincter after abdominal resection are explored. The aim of this book is to clarify the rapid advances and to offer guidelines for doctors dealing with rectal cancer. Taking into account indications, contraindications, risks, benefits and controversies, the authors offer clear and practice-oriented answers for a wide range of specialists and experts, as well as those new to the field.

Reviewer:Neal Wilkinson, MD(Roswell Park Cancer Institute)
Description:With this book, the editor and major contributing author has created a cutting edge review of the modern treatment of rectal cancer.
Purpose:The author has set forth to clearly detail modern treatment of rectal cancer. Preoperative staging, pathological evaluation, surgical treatment, and adjuvant therapy are deliberately and clearly addressed. Specific problem areas in the treatment of rectal cancer are highlighted and alternative treatment options presented.
Audience:This book is written for a surgical audience with an interest in rectal cancer. The depth and presentation of the material is excellent for surgical trainees and practicing surgeons alike. Colorectal surgeons and surgical oncology specialists will find sufficient detail and depth. Despite excellent chapters on pathology, endoscopy, and medical and radiation oncology, those in the nonsurgical specialties will not benefit from the best portions of the book which address surgical techniques.
Features:The author has compiled a wonderful array of surgical and nonsurgical chapters to cover the entire spectrum of rectal cancer. The chapters on endoscopic ultrasound and diagnostic imaging contain excellent illustrations and proper depth and detail to be useful. The sections on pathological staging, margins status and evaluation (both distal and radial) are excellent. Color photographs and clear illustrations appear throughout. Surgical techniques (APR, LAR, TME, TEM) are addressed individually in specific chapters. Complications and functional outcome are also addressed.
Assessment:In this book, the entire spectrum of rectal cancer management is addressed from epidemiology to diagnosis, staging, and treatment. Surgical as well as nonsurgical treatments are presented in clear chapters. By focusing on rectal cancer, the book eliminates all benign and inflammatory bowel topics found in most colorectal texts. I found this to be refreshing.
